Advanced Practice Provider (NP/PA) – Heart Failure

About the role

The Regional Heart, Vascular and Thoracic Institute seeks an experienced Nurse Practitioner or Physician Assistant to join the Heart Failure team at Hillcrest Hospital. The role involves managing heart‑failure patients across ICU, floor, and outpatient settings.

Key responsibilities

• Provide patient‑care management for heart‑failure patients in ICU, on the floor, and in outpatient clinics.
• Conduct comprehensive histories, physical exams, order and interpret labs, imaging, and cardiographic studies.
• Prescribe and monitor medications within scope of licensure and Cleveland Clinic policies.
• Educate patients on disease, treatment plans, and medication side‑effects.
• Participate in multidisciplinary rounds, discharge planning, and quality‑monitoring activities.

Required profile

• Current NP or PA license in the applicable state(s) and RN license.
• Minimum three years of cardiology experience (or new graduate with cardiac rotation).
• MSN degree required within three years of hire.
• National certification as a Nurse Practitioner.
• Basic Life Support certification; Advanced Cardiac Life Support or Pediatric Advanced Life Support may be required.
