Advanced Practice Provider – Medical Care at Home Urgent Dispatch

About the role

The Cleveland Clinic Health System is expanding its Medical Care at Home Urgent Dispatch team. This full‑time Advanced Practice Provider will deliver in‑home urgent evaluations, aiming to prevent emergency‑room visits and hospitalizations for patients across the West side of Cleveland and broader Northeast Ohio.

Key responsibilities

• Perform in‑home medical visits, including comprehensive histories and physical examinations.
• Conduct point‑of‑care lab testing, draw blood, place Foley catheters, and administer IV infusions and medications.
• Manage complex chronic diseases in the home setting and coordinate multidisciplinary care.
• Prescribe and monitor medications within state licensing and Cleveland Clinic policies.
• Educate patients on disease management, treatment plans, and medication side effects.

Required profile

• Graduate of an accredited APRN program (MSN required within 3 years of hire) or an A.R.C.‑approved Physician Assistant program.
• Current state licensure as NP, PA, or CNS with appropriate prescribing authority.
• Basic Life Support certification required; ACLS, PALS, or NRP may be required based on setting.
• 1‑2 years of clinical APP experience preferred, but new graduates with home‑care experience are considered.
