Transfer Center Coordinator

About the role

The Transfer Center Coordinator supports patient flow across Lehigh Valley Health Network by coordinating referrals, inpatient transfers, and emergent information. The role serves as the primary contact for physicians and medical staff, ensuring timely and appropriate patient placement.

Key responsibilities

• Monitor bed availability, assess placement priorities, and coordinate lateral transfers with other departments.
• Answer transfer center calls, transcribe interactions, and document transfer details in patient records.
• Facilitate emergent and non‑emergent transfers, consults, and conference calls between providers.
• Identify, resolve, and escalate service failures that impede patient throughput.

Required profile

• Associate’s Degree and graduate of an accredited nursing program.
• Minimum three years of acute health‑care experience.
• Ability to work with minimal supervision, manage multiple tasks, and demonstrate strong critical‑thinking and communication skills.
• Current RN or PA licensure in Pennsylvania (or compact state license) and Basic Life Support certification upon hire.

Required skills

• Proficiency with Microsoft Word, Excel, and Access.
• Basic computer keyboarding and data entry skills.
• Knowledge of hospital insurance processes and account creation.
