Telehealth Physician (MD/DO) – Remote Geriatric Care

About the role

Sailor Health is creating a holistic, preventative care model for older adults. As a Telehealth Physician you will be the clinical gateway for patients with chronic and complex conditions, delivering comprehensive virtual assessments and shaping integrated care plans.

Key responsibilities

• Conduct thorough telehealth assessments for older adults with multiple chronic conditions.
• Identify clinical risks, medication gaps, and care coordination needs.
• Collaborate with healthcare advocates, therapists, and care teams to develop and oversee patient‑centered care plans.
• Address social determinants of health alongside medical needs.

Required profile

• MD or DO, licensed in Florida, Maryland, Pennsylvania, Texas (additional states preferred).
• Board certified in Internal Medicine, Family Medicine, or a related specialty.
• Minimum of 5 hours weekly availability for telehealth visits.
• Experience caring for older adults with chronic, complex conditions.

Required skills

• Strong clinical judgment and ability to synthesize incomplete information.
• Comfort with telehealth technology and interest in leveraging AI tools for care delivery.
