Registered Nurse (RN) Hospice Case Manager – Full‑Time

About the role

The Lehigh Valley Health Network is seeking a full‑time Registered Nurse to serve as a Hospice Case Manager for patients receiving home‑based care. You will coordinate interdisciplinary care, develop and adjust care plans, and ensure high‑quality outcomes for patients and families.

Key responsibilities

• Coordinate and direct delivery of hospice care for an assigned caseload of home patients in collaboration with physicians and the interdisciplinary team.
• Initiate, review, evaluate, and revise individualized plans of care, tracking patient goals, quality metrics, and level‑of‑care transitions.
• Collect and report CMS‑required data accurately to support quality outcomes and reimbursement.
• Provide skilled assessments, treatments, and education on end‑of‑life issues, pain management, symptom control, and advance directives.

Required profile

• Graduate of an NLN‑approved nursing program with a current RN license in Pennsylvania.
• At least one year of recent direct patient care experience, preferably in home health or hospice.
• Ability to manage a caseload independently, travel to patient homes, and communicate effectively with patients, families, and the care team.
• Basic Life Support certification (AHA) and a valid driver’s license for Pennsylvania or New Jersey.

Required skills

• Knowledge of Conditions of Participation and CMS regulatory data collection.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
• Certification in Ambulatory Care Nursing (ANCC) or Certified Hospice and Palliative Nurse (CHPN) within three years is preferred.
