Registered Nurse (RN) Home Care Case Manager with $10,000 Sign‑On Bonus

About the role

The Registered Nurse (RN) Home Care Case Manager will coordinate and direct in‑home patient care for an assigned caseload, working closely with physicians and an interdisciplinary team to achieve patient goals and quality metrics. This full‑time day‑shift position includes a $10,000 sign‑on bonus.

Key responsibilities

• Assess patients, develop and revise individualized plans of care in collaboration with physicians and the care team.
• Deliver skilled nursing treatments, educate patients and caregivers, and monitor progress toward discharge or transition goals.
• Collect and report CMS data accurately to support quality outcomes and reimbursement.
• Communicate significant changes in patient status promptly to the care team.

Required profile

• Graduated from an NLN‑approved nursing program (Diploma, ADN or BSN).
• Current Pennsylvania RN license and BLS certification; driver’s license required.
• At least 1 year of recent direct‑care experience, preferably in home health or hospice.
• Ability to manage a caseload independently, travel to patient homes, and work within agency standards.

Required skills

• Basic Life Support (BLS) – American Heart Association.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
• Knowledge of Conditions of Participation and CMS regulatory data collection.
