About the role

The Home Care Admissions Nurse provides comprehensive assessments and determines the appropriate level of care for patients receiving home health or hospice services. This weekend position offers a $15,000 sign‑on bonus and works within Lehigh Valley Health Network’s award‑winning system.

Key responsibilities

• Perform initial and follow‑up subjective and objective assessments covering physical, psychological, and environmental factors.
• Complete CMS‑required data collection with high accuracy and formulate individualized plans of care based on physician orders and interdisciplinary input.
• Communicate significant changes in patient status promptly to physicians and the care team.
• Provide patient and caregiver education on end‑of‑life issues, palliative care, chronic disease management, pain control, and discharge planning.

Required profile

• Graduate of an NLN‑approved nursing program with current RN licensure in Pennsylvania.
• At least one year of recent direct‑care experience in a healthcare setting.
• Ability to work independently, travel to patient homes, and manage time effectively.
• Current BLS certification (American Heart Association) and a valid driver’s license.

Required skills

• BLS (Basic Life Support) certification
• Proficiency with multiple EMR platforms
• Knowledge of CMS regulations and Conditions of Participation
