About the role
Lehigh Valley Health Network is seeking a licensed Physical Therapist to provide home‑care services in Northampton. The role involves evaluating, planning, and delivering therapy to restore function and improve quality of life for patients.
Key responsibilities
- Establish and implement individualized physical therapy plans of care using evidence‑based techniques such as therapeutic exercises, manual techniques, functional mobility training, and modalities.
- Perform patient assessments, reassessments, and modify treatment plans as needed while documenting progress and communicating with physicians and the care team.
- Provide supervision in accordance with regulatory requirements and ensure cost‑effective care aligned with departmental billing guidelines.
Required profile
- Bachelor’s Degree in Physical Therapy and Pennsylvania state licensure (required upon hire).
- Ability to travel to patient homes and perform physical demands such as lifting 40‑50 lb, standing, walking up to 8‑10 miles per shift, and performing CPR.
