Patient Care Assistant – Med/Tele 2 – Days

About the role

The Patient Care Assistant works under the direct supervision of a licensed nurse to ensure patient safety and deliver basic care. This position involves constant observation, timely communication of changes, and assistance with daily activities in a clinical setting.

Key responsibilities

• Support the plan of care as delegated by the staff nurse, within the scope of verified skills.
• Maintain a clean and safe patient care environment.
• Communicate patient needs effectively with patients, families, and multidisciplinary team members.
• Complete documentation of all patient‑related tasks.
• Adapt behavior to respect patient privacy and preferences, requesting permissions as needed.
• Perform other related duties as assigned.

Required profile

• High school diploma or equivalent.
• Preferred: at least 3 months of personal care experience in a home, nursing home, skilled nursing, or hospital setting.
• Certifications: Basic Life Support (BLS) and Crisis Prevention and Intervention (CPI) to be obtained within 30 days of hire.

Required skills

• Computer proficiency for data entry and information retrieval.
• Ability to use Windows‑style applications and role‑specific software.
• Basic Life Support (BLS) certification.
• Crisis Prevention and Intervention (CPI) certification.
