Outreach Specialist

About the role

Habitat Health is seeking an Outreach Specialist to expand awareness of its PACE program and build strong relationships with healthcare providers, community organizations, and referral partners. You will play a pivotal role in guiding prospective participants through the enrollment journey, ensuring a seamless experience from referral to enrollment.

Key responsibilities

• Develop and maintain trusted relationships with providers, senior living communities, post‑acute facilities, and other referral sources.
• Design and execute outreach strategies that increase program visibility and generate qualified referrals.
• Represent Habitat Health at community events, health fairs, presentations, and networking sessions.
• Act as case manager for prospective participants, reviewing applications, processing documentation, and managing communications.
• Identify new referral opportunities and refine outreach tactics using market insights and performance data.
• Maintain accurate referral and activity records in the CRM and other internal systems.
• Monitor outreach metrics and use data to prioritize efforts and drive continuous improvement.

Required profile

• Strong interpersonal skills with a proven ability to build and sustain community partnerships.
• Excellent organizational abilities and attention to detail in managing enrollment documentation.
• Data‑driven mindset for tracking performance and optimizing outreach activities.

Required skills

• Experience using a CRM system to track referrals and relationships.
