Center Manager – PACE Program

About the role

Habitat Health is seeking a Center Manager to lead its PACE (Program of All‑Inclusive Care for the Elderly) Center in West Covina. The manager will ensure safe, efficient, participant‑centered delivery of medical and social services while supporting a multidisciplinary team.

Key responsibilities

• Oversee daily center operations, including participant flow, staffing, scheduling, and service coordination.
• Participate actively in the interdisciplinary team (IDT), ensuring timely assessments, care planning, documentation, and collaboration across clinical and social disciplines.
• Maintain compliance with PACE, CMS, DHCS, ADHC, and state regulations, monitoring documentation accuracy and audit readiness.
• Lead, coach, and manage center staff, fostering a culture of accountability and teamwork.
• Track operational and quality performance metrics, identify gaps, and implement improvements to meet service level agreements.
• Collaborate with the Medical Director, Clinical Services Director, and Practice Operations leadership to achieve performance goals.

Required profile

• Proven experience managing a healthcare or senior‑services center.
• Strong knowledge of PACE, CMS, and relevant state regulations.
• Demonstrated leadership, staff development, and performance‑management skills.
• Excellent organizational, problem‑solving, and communication abilities.
• Commitment to delivering participant‑centered, high‑quality care.
