Enrollment Specialist

About the role

Habitat Health is seeking an Enrollment Specialist to manage the end‑to‑end enrollment journey for prospective participants in its PACE program. The role works closely with Outreach Specialists, Enrollment RN, and the interdisciplinary team to guide participants and caregivers through enrollment, remove barriers, and ensure a participant‑centered experience.

Key responsibilities

• Build trusted relationships with prospective participants and caregivers to support enrollment growth.
• Act as case manager for enrollment, reviewing, processing, and updating applications and required documentation.
• Coordinate enrollment cases, managing communications and follow‑up activities in a timely and accurate manner.
• Partner with Outreach team, Enrollment RN, IDT, and center leaders to deliver a coordinated enrollment experience.
• Conduct enrollment visits in participants’ homes, centers, or virtually to assess needs and support eligibility activities.
• Identify and help resolve barriers to enrollment by proactively coordinating resources and maintaining communication.

Required profile

• Strong ability to build and maintain trusted relationships with participants and caregivers.
• Experience managing case‑based processes and documentation.
• Excellent communication and coordination skills across multidisciplinary teams.
• Comfort conducting in‑person or virtual visits to assess participant needs.
