Occupational Therapist – Home Health

About the role

A premier therapist‑led national healthcare organization is seeking a full‑time Occupational Therapist for its growing Home Health division in the East Valley of Phoenix. You will deliver patient‑centered care in patients' homes, managing your own caseload while collaborating with an interdisciplinary team.

Key responsibilities

• Assess clients' physical, emotional, and cognitive abilities using clinical observations and standardized tools.
• Develop and implement personalized treatment plans to improve functional independence.
• Educate clients and families on adaptive equipment, home modifications, and activity adaptations.
• Document evaluations, progress notes, and treatment plans accurately and timely.
• Advocate for clients’ needs, connecting them with community resources and necessary accommodations.

Required profile

• Degree in Occupational Therapy from an accredited institution.
• Valid Arizona state licensure (or license‑eligible).
• Strong clinical assessment, diagnostic, and treatment planning abilities.
• Physical capacity to lift patients, stand for extended periods, and perform home‑based therapy tasks.
