Case Manager – Behavioral Health

About the role

The Case Manager supports the Clinical Coordinator in delivering high‑quality substance‑use treatment to adults, including veterans, in a fast‑paced behavioral health setting. This full‑time, on‑site position involves managing a client caseload, providing individual and group interventions, and coordinating care and discharge planning.

Key responsibilities

• Develop, monitor, and document individual treatment plans and client progress.
• Maintain a regulated caseload, completing progress notes, evaluations, and discharge plans on time.
• Provide individual and group crisis interventions and facilitate coordination of after‑care services.
• Assist clients with applications for government assistance, court obligations, and referrals to external agencies.

Required profile

• High school diploma or GED; 1‑2 years related experience or equivalent education.
• Must meet Arizona Department of Health Services personnel requirements (age 21+, valid fingerprint clearance, CPR/First Aid/Narcan training, negative TB test, clean motor vehicle record preferred).
• Ability to work days, nights, weekends, and holidays with occasional travel between sites.

Required skills

• Proficiency with Microsoft Office and electronic medical record (EMR) systems.
• Strong written and verbal communication skills for interaction with internal and external audiences.
