About the role

We are seeking an LPN Care Coordinator for our GI Clinic to support data collection and the delivery of health care services. The role assists other members of the health care team in providing high‑quality, personalized patient care and maintains positive relationships across the team.

Key responsibilities

  • Oversee the disease registry database.
  • Conduct pre‑visit chart reviews of patients.
  • Coordinate care across the continuum.
  • Participate in quality improvement activities.
  • Perform other related duties as required.

Required profile

  • Graduate of an accredited practical nursing program.
  • Minimum 2 years of ambulatory care experience (3 years preferred).
  • Current LPN license in the state of practice.
  • Proficiency with computers, software, and web‑based applications.
  • Effective verbal and written communication skills.
  • Strong organizational, decision‑making and delegating abilities.
  • Positive interpersonal and public relation skills.
  • Leadership ability and capacity to set priorities.
