Licensed Psychiatric Case Manager (LCSW/LPC)

About the role

The Licensed Psychiatric Case Manager provides mental health assessment, diagnosis, treatment, and crisis intervention for adult and child members, coordinating care to maintain patients in the community with the least restrictive environment.

Key responsibilities

• Oversee utilization management, including service authorizations and denial/appeal processes.
• Ensure clear, timely discharge planning documentation aligned with treatment team recommendations.
• Conduct clinical assessments and provide case management and care coordination.
• Act as liaison between patients, referral sources, and funders, and manage voluntary/involuntary commitment processes per Pennsylvania regulations.

Required profile

• Master’s degree in Social Work, Counseling, or related field.
• Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Social Worker (LSW) in Pennsylvania.
• Ability to remain calm in stressful situations and knowledge of community resources.
