Hospice Case Manager (RN) – Home Hospice

About the role

The RN Case Manager provides intermittent skilled nursing services for hospice patients, coordinating care with multidisciplinary teams and supervising hospice aide staff.

Key responsibilities

• Admit patients eligible for hospice services under physician orders.
• Assess patient needs, set mutually agreed goals, and update care plans continuously.
• Communicate patient status and interdisciplinary needs to clinical leadership and physicians.
• Complete informational visits, obtain consents, and ensure use of the 4Ms framework (What Matters, Medications, Mentation, Mobility).

Required profile

• Current, unencumbered registered nurse license in the state of practice or NLC‑eligible.
• Valid driver’s license with a good driving record.
• Ability to work in a field setting and make sound nursing judgments.

Required skills

• Skilled nursing assessment and care planning.
• Supervision of hospice aide staff.
