Transplant Social Services Consultant

About the role

The Transplant Social Services Consultant provides specialized social services support to transplant patients and families within the Kids First/Headstart program. This position works closely with clinical teams to ensure comprehensive psychosocial care throughout the transplant process.

Key responsibilities

• Assess psychosocial needs of transplant candidates and donors.
• Develop and implement individualized care plans.
• Coordinate resources, counseling, and financial assistance.
• Collaborate with multidisciplinary teams to facilitate patient education and discharge planning.
• Maintain accurate documentation in compliance with hospital policies.

Required profile

• Valid social work or related licensure.
• Experience in transplant or complex medical settings preferred.
• Strong communication and advocacy skills.
• Ability to work independently and as part of a team.

Required skills

• Knowledge of transplant protocols and patient navigation.
• Proficiency in clinical documentation systems.
