Telehealth Nurse Practitioner (Remote)

About the role

This fully remote position enables a Nurse Practitioner to deliver telehealth care for Medicare and Medicaid members, earning $600–$720 per day. The role is structured for NPs licensed in multiple states, comfortable with HEDIS and HCC workflows, and able to complete high‑quality virtual assessments on a flexible schedule.

Key responsibilities

• Conduct comprehensive health assessments via telehealth visits.
• Review medical history, medications, and preventive care needs.
• Document HCC risk‑adjustment findings accurately.
• Identify and help close HEDIS quality‑measure care gaps.
• Use ICD‑10 and CPT II codes to support compliant documentation.
• Provide clear care plans and follow‑up guidance to members.
• Complete timely documentation that supports value‑based care workflows.
• Follow structured processes to ensure consistent visit flow and documentation quality.

Required profile

• Active Nurse Practitioner license in at least two eligible states (e.g., PA, WA, TX, etc.).
• Minimum 1 year experience in Family Medicine or Internal Medicine.
• Experience caring for Medicare and Medicaid populations.
• Familiarity with HEDIS and risk‑adjustment workflows.
• Ability to work at least 24 hours per week, preferably on weekdays during daytime.
