About the role

Join Jefferson Health at Home by BAYADA as a Wound, Ostomy and Continence Nurse (WOCN) providing skilled, compassionate care to patients in the comfort of their own homes across King of Prussia, PA.

Key responsibilities

• Deliver 1:1 skilled nursing, medication administration, wound, ostomy and continence care according to individualized care plans.
• Conduct client assessments, provide education to patients and families, and monitor conditions.
• Perform wound care consultations and provide oversight to ensure optimal outcomes.

Required profile

• Active RN license in Pennsylvania.
• Current WOCN certification.
• Ability to travel to assigned cases.
• Calm, reassuring presence with strong assessment and communication skills.
