Treatment Coordinator – Outpatient Mental Health

About the role

Community Wellness Outpatient Mental Health Center is seeking a licensed behavioral health professional to lead care, support progress, and keep clients connected to treatment. The Treatment Coordinator will manage an assigned caseload, provide weekly clinical support, coordinate referrals, and ensure services align with client needs and program standards.

Key responsibilities

• Provide weekly clinical support to assigned clients, including assessments, treatment planning, and progress monitoring.
• Coordinate care with therapists, psychiatric providers, case managers, and community resources, tracking referrals for services such as housing, substance use treatment, and primary care.
• Maintain accurate documentation in the electronic medical record, ensuring compliance with agency, payer, and regulatory standards.
• Serve as Treatment Team Lead, offering coaching, feedback, and support to team members and helping resolve barriers to service delivery.

Required profile

• Active Maryland license or certification (e.g., LCPC, LCSW, LCMFT, LCADC, APRN-PMH, Licensed Psychologist, etc.).
• Experience in outpatient or community mental health settings, with strong skills in assessment, treatment planning, and care coordination.
• Knowledge of trauma‑informed, person‑centered, and recovery‑oriented care approaches.
• Reliable transportation for onsite or community‑based service needs.

Required skills

• Proficiency with electronic medical record systems.
• Basic computer applications (e.g., email, word processing).
