Success Coach (Case Manager) - Targeted Case Management

About the role

Families & Youth Innovations Plus (FYI+) is hiring a full-time, hourly, non-exempt Success Coach (Case Manager) for its Targeted Case Management (TCM) program in Las Cruces. The coach provides specialized case management tailored to each client's needs, creates programs that benefit clients and educates them about community resources so that services are delivered in a timely, coordinated way. In-person support is required in settings such as homes, schools, courts, offices and street outreach locations, and on-camera online participation is also expected.

Key responsibilities

• Coordinate services between providers and participants
• Provide case management and skill-building services to youth in the program, including housing case management
• Maintain individual case records and administer intake forms, assessments, intake packets and service plans
• Educate participants about community resources and create programs that benefit clients and the community
• Attend community meetings and conduct field visits, including in educational institutions, workplaces, detention facilities and other settings
• Facilitate life skills in individual and group settings and take part in youth activities, which may include overnight trips

Required profile

• Bachelor's degree in a human services field plus one or more years of relevant experience, or
• Associate's degree in human services plus at least three years of hands-on mental health experience, or
• High school diploma or equivalent with at least five years of supervised behavioral health experience (subject to approval)
• Criminal records check with CYFD and current CPR/First Aid and CPI certifications
• Bilingual English/Spanish preferred

• Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) or Adobe Acrobat, or willingness to learn them
