Substance Use Navigator – Emergency Department (WestBank)

About the role

The Substance Use Navigator (SUN) serves as a front‑line resource in the Emergency Department (ED), engaging patients with substance use disorders (SUD) through compassionate, non‑judgmental support. This role facilitates rapid linkage to treatment, harm‑reduction services, and community‑based resources to improve patient outcomes and reduce repeat ED utilization.

Key responsibilities

• Conduct bedside engagement of ED patients with substance‑use concerns using trauma‑informed, person‑centered approaches.
• Perform SBIRT activities, educate patients and families on SUD, harm‑reduction, and treatment options.
• Facilitate real‑time referrals and warm handoffs to inpatient/outpatient programs, MAT providers, peer support, and community resources.
• Coordinate with ED clinicians, social workers, case managers, and behavioral‑health teams to support care plans.
• Document all interactions and referrals accurately in the electronic health record (EHR) and assist with follow‑up outreach.

Required profile

• High School Diploma/GED required; Associate’s or Bachelor’s in Social Services, Behavioral Health, Public Health or related field preferred.
• Minimum 1 year experience in healthcare, behavioral health, social services, community outreach, or related area.
• Personal lived experience of substance use and sustained recovery (or supporting a loved one) is strongly encouraged.
• Preferred certifications: Certified Peer Support Specialist, Community Health Worker, SBIRT training, crisis prevention‑de‑escalation training.

Required skills

• Proficiency with electronic health records (EHR) documentation.
• Basic computer applications and accurate documentation workflows.
