Substance Use Disorder Case Manager / Outreach Specialist

About the role

The Substance Use Disorder Case Manager / Outreach Specialist provides comprehensive case management for court, DCFS, probation and self‑referred clients, supports outreach efforts, and promotes drug and alcohol services within the community.

Key responsibilities

• Conduct assessments, individual sessions, treatment planning, urine screenings, intakes and crisis interventions.
• Maintain client charts to meet county and state requirements and submit documentation within 48 hours.
• Recruit new youth and adult clients through field‑based outreach and community events.
• Provide counseling, manage a substantial caseload, and transport clients when needed.
• Complete a minimum of 20 billable hours per week (excluding group sessions) and submit weekly reports.

Required profile

• Registered through CCAPP, CAADE, or a comparable certifying body.
• At least one year of experience in addiction treatment and formal education in SUD.
• Strong communication skills, high‑energy outreach ability, and bilingual Spanish is a plus.

Required skills

• Proficiency with Microsoft Office.
• Ability to write individual and group progress notes and treatment plans meeting county standards.
• Experience facilitating groups and observing client drug testing.
