Lead Case Manager – Homeless Services

About the role

The Lead Case Manager will guide a multidisciplinary team delivering outreach, case management, and coordinated health and housing services to individuals experiencing homelessness in Los Angeles.

Key responsibilities

• Engage directly with homeless individuals in street‑based and walk‑in settings, building trust and linking them to healthcare, housing, food, and other essential resources.
• Develop and implement individualized care plans, provide ongoing case management, crisis intervention, and advocacy to ensure clients access needed services.
• Lead resource development by creating new partnerships and expanding housing and health service options for clients.
• Collaborate with internal programs such as Enhanced Care Management, Behavioral Health, and Substance Use departments.
• Document client interactions in EPIC and HMIS, maintain accurate records, and generate regular reports.

Required profile

• Bachelor’s degree in social work, human services, public health, nursing or related field, or equivalent experience.
• Strong knowledge of Los Angeles housing resources and experience in case management.
• Excellent communication, interpersonal, and crisis‑management skills.
• Ability to handle multiple cases in a fast‑paced environment while maintaining confidentiality.

Required skills

• Proficiency with Electronic Medical Record systems, specifically EPIC.
• Experience using Homeless Management Information Systems (HMIS).
• Advanced use of the MS Office Suite.
