Street Outreach Coordinator

About the role

The Street Outreach Coordinator connects unhoused individuals with essential services and supports their entry into mainstream programs across East Washington County. The role involves community‑based outreach, referral triage, and ongoing case follow‑up.

Key responsibilities

• Engage actively unhoused and unsheltered individuals, linking them to shelter, basic needs, and mental health resources.
• Respond to and triage referrals from emergency rooms, jails, shelters, and other sources.
• Develop short‑term solutions for barriers such as transportation or emergency assistance.
• Create individualized service plans, enroll clients in the Homeless Management Information System (HMIS), and track services provided.
• Collaborate with agencies, attend community meetings, and provide training on homeless and mental‑health resources.

Required profile

• High school diploma or equivalent.
• Experience working with adults with severe mental illness and homeless populations.
• Valid driver’s license, reliable vehicle, and insurance.
• Strong motivational interviewing and professional representation skills.

Required skills

• Computer literacy for data entry and HMIS use.
• Ability to identify service barriers and develop short‑term solutions.
