Speech Therapist – Home Health

About the role

Commonwealth Home Care seeks a licensed Speech-Language Pathologist to provide high‑quality, evidence‑based speech therapy services to clients in their homes across Norfolk, VA. You will work closely with physicians and interdisciplinary teams to enhance patients' communication and swallowing abilities.

Key responsibilities

• Evaluate clients’ speech and swallowing function, develop and adjust treatment plans in collaboration with physicians.
• Conduct comprehensive assessments, including OASIS assessments, and document progress within 72 hours of each visit.
• Educate and counsel clients and families on therapy techniques and home care strategies.
• Coordinate visit schedules with the Director of Nursing and participate in case conferences.

Required profile

• Master’s degree in Speech‑Language Pathology (Bachelor’s preferred).
• Virginia SLP license, ASHA membership preferred, and current CPR certification.
• At least one year of institutional experience and ability to provide references.
• Reliable transportation, valid driver’s license, and proof of insurance.
