Occupational Therapist – Part‑time or Full‑time

About the role

Commonwealth Home Care is seeking a licensed Occupational Therapist to deliver skilled OT services to clients in their homes across the Chesapeake area. You will work independently and collaborate with the care team to enhance patients' function, safety, and independence.

Key responsibilities

• Obtain medical history, evaluate clients and home environments, and develop individualized care plans.
• Provide OT interventions, teach use of adaptive equipment, and supervise COTAs as needed.
• Document clinical notes on the day of service and update care plans within 72 hours.
• Coordinate visit schedules with nursing leadership and arrange outpatient services when required.

Required profile

• Graduate of an accredited OT program and licensed in Virginia.
• Minimum one year of institutional or home‑health OT experience.
• Current CPR certification (or ability to obtain before start).
• Reliable transportation, valid driver’s license, and proof of car insurance.

Required skills

• Patient evaluation and functional assessment.
• Clinical documentation and progress note writing.
• Use and instruction of wheelchairs, braces, splints, and orthotic devices.
• Supervision of Occupational Therapy Assistants.
