Physical Therapist Assistant

About the role

Commonwealth Home Care is seeking a Physical Therapist Assistant to provide skilled therapy services to clients in their homes across Norfolk, VA. You will work under the direction of a Registered Physical Therapist to help patients relieve pain, improve function, and maintain independence.

Key responsibilities

• Assist the PT in evaluating patients, developing and updating rehabilitative plans of care.
• Perform therapeutic exercises, use self‑help devices, and document clinical progress within 72 hours.
• Teach exercise programs to patients and families and report findings to the PT.
• Participate in case conferences, staff meetings, and in‑service training.

Required profile

• Graduate of an approved PTA program (minimum two‑year course).
• Licensed or registered PTA in Virginia with at least one year of home‑health experience.
• Current CPR certification and ability to pass background checks.
• Reliable transportation, valid driver’s license and insurance.

Required skills

• CPR certification
• Working knowledge of Medicare regulations (plus)
