Physical Therapist – Flexible Schedule

About the role

Commonwealth Home Care seeks a compassionate Physical Therapist to deliver skilled home‑based therapy, helping clients relieve pain, restore function, and maintain performance. Flexible scheduling options—including full‑time, part‑time, or PRN—are available for the right candidate.

Key responsibilities

• Conduct comprehensive assessments, including OASIS, and develop individualized treatment plans in collaboration with physicians.
• Provide hands‑on therapy services, teach use of mobility aids, and supervise Physical Therapy Assistants.
• Document evaluations, treatments, and progress notes promptly; communicate changes to physicians and team members.
• Participate in case conferences, staff meetings, and in‑service programs.

Required profile

• Graduate of an accredited Physical Therapy program and licensed in Virginia.
• Minimum one year of institutional or home‑health experience with current CPR certification.
• Reliable transportation, valid driver’s license, and ability to pass background checks.

Required skills

• Proficiency with PT equipment and home‑health documentation tools (e.g., Kinnser/Wellsky).
• Experience conducting OASIS assessments and understanding Medicare regulations.
