Social Worker - Social Service Care Manager

About the role

Catholic Housing and Community Services is looking for a part-time (20 hours per week) Social Service Care Manager for an older adult affordable housing site in a far Northeast Philadelphia neighborhood. The role is on-site and central to the residential community, engaging older adults, using digital case management tools and building social, recreational and health-related services in an age-affirming, relationship-centered way.

Key responsibilities

• Manage a caseload of older adult households in an independent apartment community.
• Assess each resident for strengths and barriers to aging in place, including physical and cognitive health, ability to manage daily living tasks, mobility, access to benefits and economic stability.
• Explore intergenerational partnerships and programming with the adjacent Catholic High School.
• Work with residents to create resident-centered care plans with interventions and resources.
• Maintain resident files in the social service database, including case notes, support contacts, assessments, reassessments and care plans.
• Advocate with community-based providers to remove barriers to services and create a supportive community atmosphere through onsite events such as holiday and birthday parties, presentations and vaccine and legal clinics.
• Track intervention and group participation data and use data analysis tools to improve program planning.
• Answer external calls for information and resources.

Required profile

• Master's degree in Social Work, Gerontology or a related field from an accredited university.
• At least three years of experience working with seniors.
• Knowledge of federal and state benefit programs and their eligibility requirements.
