Case Manager - Housing and Homeless Services

About the role

Catholic Charities of Philadelphia is hiring a Case Manager for its McAuley House program in Housing and Homeless Services. The position is full-time, involves evenings and weekends, and sits within a leadership role supervised by the Program Director/Supervisor. It provides case management services to all residents of McAuley House under Catholic Social Services standards and the requirements of contracting agencies.

Key responsibilities

• Complete intake paperwork and assessments for new and assigned residents and orient them to the program.
• Develop and maintain case files and quarterly service plans with goals, objectives and timelines, and monitor progress in weekly sessions.
• Maintain a list of services and resources and refer residents to them; assist with medical, mental health, social security and county assistance appointments, and transport residents when approved.
• Plan weekly individual case management meetings and document progress notes at least twice a month, including housing information.
• Monitor residents' health and prescribed medications, and track refills.
• Assist residents with housing applications and provide follow-up to former residents.
• Help the supervisor with data collection, reporting and program needs, and help approve new admissions.
• Collaborate with the supervisor on recruiting volunteers for life skills groups, scheduling chores and inspecting bedrooms bi-weekly.
• Work a flexible schedule, rotate monthly and cover on-call weekends answering questions about residents.

Required profile

• BA degree.
• Two years of social service experience.
• Interest in serving the homeless population and people living with HIV/AIDS.
• Driver's license.
