Social Worker MSW

About the role

The Social Worker MSW provides culturally competent assessments and coordinates social service interventions for patients and families at Martin Health North. Working within a multidisciplinary team, the role supports patients through crisis intervention, discharge planning, and ongoing care coordination.

Key responsibilities

• Conduct comprehensive biopsychosocial and spiritual assessments to develop individualized care plans.
• Respond to campus‑wide referrals on the same working day and identify significant psychosocial issues.
• Utilize a multidisciplinary approach to ensure continuity of care and maximize effectiveness of interventions.
• Facilitate team conferences for complex cases and provide crisis intervention techniques.
• Document progress notes, problem lists, and interdisciplinary education records.
• Coordinate inpatient and outpatient dialysis arrangements and assist with discharge planning for patients with complex needs.

Required profile

• Master's Degree in Social Work.
• At least one year of clinical experience, preferably in a hospital or healthcare setting.
• Basic computer proficiency with Windows‑based software.
• Experience in crisis intervention, psychosocial counseling, and utilization management.
• Strong organizational, prioritization, and leadership abilities.

Required skills

• Proficiency with Windows operating system and related software applications.
