Social Worker LCSW – Abdominal Transplant (Full Time)

About the role

This position provides advanced clinical social work services within the abdominal transplant program at Ochsner Health. The Social Worker collaborates with multidisciplinary teams to support patients and families throughout the transplant journey.

Key responsibilities

• Interview and assess patients, developing and coordinating comprehensive care plans.
• Identify and connect patients and families with community and hospital resources.
• Maintain accurate patient records and communicate critical information to the medical team.
• Participate in on‑call rotation and ensure continuity of care.
• Adapt communication style to respect patient privacy and cultural needs.

Required profile

• Master’s degree in Social Work (MSW).
• Minimum 5 years of related experience, preferably in medical social work.
• Current Licensed Clinical Social Worker (LCSW) credential in the state of practice.

Required skills

• Strong knowledge of disease entities, medical terminology, and psychosocial impacts of illness.
• Excellent psychotherapeutic and interpersonal skills.
• Advanced understanding of managed care programs (Medicare, Medicaid, commercial insurance).
• Proficiency with computers, software, and web‑based applications.
• Effective verbal and written communication for diverse audiences.
