About the role

Cleveland Clinic Health System is seeking a Social Worker to provide psychotherapeutic and care management support to patients admitted to the Behavioral Health Center at Indian River Hospital. This PRN position works flexible shifts, including weekends and nights, to meet complex psychosocial needs.

Key responsibilities

Coordinate treatment and discharge planning for psychiatrically acute patients.
Develop and implement interdisciplinary treatment plans based on comprehensive psychosocial assessments.
Participate in interdisciplinary rounds and communicate discharge status to the care team.
Supervise and train social work graduate students and interns.
Advocate for patients and families, intervene in cases of abuse or neglect, and coordinate legal resources.
Provide consultation to medical floors, ER, and outpatient areas for complex psychosocial cases.

Required profile

Master's Degree in Social Work, Counseling or related field.
No record of substantiated ethics or patient abuse reports.
High degree of manual dexterity to produce materials on a PC.
