Social Worker - Care Management

About the role

UChicago Medicine AdventHealth is hiring a full-time day-shift Social Worker Care Management in La Grange, Illinois. The role supports patients and families through discharge planning, crisis intervention and care coordination across inpatient, observation and emergency departments.

Key responsibilities

• Provide grief counseling, disease adjustment support, crisis intervention, goals of care planning and de-escalation services for patients as appropriate.
• Assess patients and families holistically for discharge planning needs, including prior functioning, support systems, finances and psychosocial factors, to avoid delays.
• Review medical records and incorporate clinical, social and financial factors into the transition of care plan.
• Develop discharge plans with contingency plans, arranging post-acute care and community care for social needs.
• Use technology and best practices to communicate with post-acute providers so records are complete and discharge reconciliation is accurate.
• Take part in multi-disciplinary rounds and organize care conferences with the team.
• Educate patients and families on the emotional, social and financial impact of illness, advocate for their choices and rights, and assess readmitted patients.
• Document discharge planning evaluations, barriers to care and patient needs, and coordinate payer authorizations for post-acute care.

Required profile

• Master's degree (required).
• 2+ years of social work experience (required); 2+ years of care management experience preferred.
• State LSW or LCSW license (required).
• ACM or CCM certification preferred.

Required skills

• Psychosocial assessment, discharge planning, utilization management and care management.
• Outlook e-mail and electronic medical records.
• Preferred: knowledge of state and federal care management guidelines and data analysis skills.
