Rural Health Transformation Program Navigator / Community Health Worker

About the role

The Rural Health Transformation Program (RHTP) Navigator/Community Health Worker joins a Local Community Hub in Missouri’s ToRCH Care model. Reporting to the Hub Program Manager, the role screens residents, coordinates referrals, and removes barriers to health, behavioral health, and social services.

Key responsibilities

• Conduct standardized intake and needs screenings, documenting findings and referrals.
• Coordinate warm handoffs among healthcare and community partners, confirming service receipt.
• Manage an assigned caseload, following up until services are completed or care is transitioned.
• Engage residents through calls, field visits, and community outreach, providing health education.
• Address barriers such as transportation, medication access, insurance, food resources, and digital access, escalating complex issues.

Required profile

• Associate’s degree in community health, public health, social services, behavioral health, human services, or related field.
• Valid driver’s license with ability to travel routinely across a multi‑county area.
• Experience in community health, care coordination, case management, or related fields, especially in rural or high‑need populations.

Required skills

• Proficiency with computers and digital documentation systems.
• Strong organizational and follow‑up abilities for multiple cases.
