RN Utilization Review / Patient Care Coordinator

About the role

Ashe Memorial Hospital in West Jefferson, NC, is hiring a Registered Nurse (RN) as Utilization Review / Patient Care Coordinator within its Care Coordination department. The role oversees the Swing Bed (Transitional Care) Program, ensuring appropriate resource use, regulatory compliance and safe, efficient care transitions from admission through discharge. It is a full-time, day-shift position (Monday to Friday) with occasional evenings and weekends as needed.

Key responsibilities

• Coordinate and manage the Swing Bed (Transitional Care) Program
• Review admissions for medical necessity and regulatory compliance
• Conduct utilization reviews to optimize length of stay and resource use
• Manage internal and external referrals and build provider relationships
• Collaborate with physicians, case management and interdisciplinary teams
• Support discharge planning for safe and timely transitions of care
• Handle pre-authorizations and follow up on denials
• Ensure compliance with regulatory, accreditation and hospital standards
• Maintain accurate documentation and reporting

Required profile

• Current Registered Nurse (RN) license in North Carolina
• Basic Life Support (BLS) certification (required)
• Utilization Review experience (required)
• Preferred: experience with Swing Bed or transitional care programs, case management or discharge planning, and hospital-based clinical experience
