RN Outpatient Care Manager - Cardiac

About the role

The RN Outpatient Care Manager-Cardiac provides comprehensive case management for complex cardiac patients, coordinating care across the continuum and ensuring seamless service delivery.

Key responsibilities

• Assess patients' healthcare needs, identify barriers, and develop comprehensive treatment plans with specific goals.
• Collaborate with primary care providers, multidisciplinary teams, caregivers, and community resources to implement and adjust care plans.
• Coordinate referrals, appointments, and resources to manage disease processes in the home environment.
• Provide disease‑based education, self‑management support, and community resources to reduce hospital admissions.
• Monitor plan effectiveness, identify quality issues, and report to leadership.

Required profile

• Graduate of an accredited nursing program; Bachelor’s preferred.
• At least 3 years of clinical experience, including documentation in an electronic medical record and use of Microsoft Office.
• Current RN license in the state of practice; case manager certification preferred.

Required skills

• Proficiency with electronic medical record systems.
• Strong computer, software, and web‑based application skills.
• Effective verbal and written communication for presenting information professionally.
