RN On-Call After Hours Hospice Nurse

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based care. As the Hospice RN On-Call, you provide after-hours clinical support, triage patient needs and coordinate care so that patients receive continuity and quality outcomes. The role covers remote and field-based nursing support, assessment of patient conditions and collaboration with interdisciplinary teams, serving the Woburn and surrounding area.

Key responsibilities

• Provide on-call nursing support, triage patient needs and determine appropriate interventions
• Assess patient conditions by phone and/or in person and escalate care as needed
• Coordinate with physicians, care teams and caregivers for timely care delivery
• Deliver patient and caregiver education on symptom management and care plans
• Maintain accurate and timely documentation of patient interactions and clinical decisions

Required profile

• Current RN license for the state(s) of assignment
• 1+ year of nursing experience, preferably in hospice, home health or acute care
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

What we offer

• Salary of $75,450 to $113,174 annually for full-time employment, plus a 10K sign-on bonus

• Shift pattern of 7 days on and 7 days off, Monday to Friday 5pm to 8am and 24 hours on both Saturday and Sunday
