RN Nurse Navigator – Urology Clinic

About the role

The RN Nurse Navigator supports patients in the Urology Clinic by coordinating care across settings, communicating with multidisciplinary teams, and ensuring smooth transitions from admission to discharge. This full‑time position is based onsite in New Orleans and requires a compassionate, detail‑oriented registered nurse.

Key responsibilities

• Provide continuity of care by managing patient transitions between acute, post‑acute, and community settings.
• Collaborate with physicians, caregivers, and multidisciplinary team members to develop and update patient‑centered care plans.
• Facilitate discharge planning, identify barriers to care, and work to reduce readmissions.
• Serve as patient advocate, communicating care options and coordinating post‑discharge follow‑up.

Required profile

• Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred.
• Minimum 2 years of nursing experience, preferably in a clinical or specialty setting.
• Current RN license in the state of practice and Basic Life Support certification.
• Ability to work flexible schedules, including weekends, holidays, and on‑call availability.

Required skills

• Proficiency with computers, software, and web‑based applications.
• Basic Life Support (BLS) certification from the American Heart Association.
