RN Navigator – Pulmonary Clinic – Lung Nodule

About the role

The RN Navigator in the Pulmonary Clinic supports patients with lung nodules throughout their care journey. Acting as a liaison among patients, caregivers, providers, and payers, the role ensures smooth transitions and coordinated follow‑up.

Key responsibilities

• Provide continuity of care by managing transitions between care settings and developing patient‑centered care plans.
• Collaborate with physicians and multidisciplinary teams to communicate and implement treatment plans.
• Identify barriers to care, facilitate discharge planning, and work to reduce readmissions.
• Serve as patient advocate, communicating with caregivers, post‑acute services, and third‑party payers.
• Participate in process improvement initiatives and maintain compliance with regulatory standards.

Required profile

• Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred.
• Minimum 2 years of nursing experience.
• Current RN license in the state of practice and Basic Life Support (BLS) certification.
• Ability to work flexible schedules, including weekends, holidays, and on‑call availability.

Required skills

• Proficiency with computers, software applications, and web‑based tools.
• Basic Life Support (BLS) certification.
• Current registered nurse (RN) license.
