About the role
The RN Navigator serves as the primary liaison between patients with inflammatory bowel disease, their caregivers, and the multidisciplinary care team at the Jeff Hwy clinic. This role coordinates care transitions, discusses alternative options, and supports discharge planning to ensure seamless, cost‑effective outcomes.
Key responsibilities
- Provide continuity of care by managing smooth transitions between care settings.
- Develop relationships with patients and the multidisciplinary team to navigate treatment and follow‑up.
- Manage high‑risk, complex patient cases to minimize readmissions.
- Collaborate with physicians and the care team to communicate and implement patient‑centered care plans.
- Identify barriers to care and propose process improvements for patient navigation.
Required profile
- Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred.
- Minimum two years of nursing experience.
- Current registered nurse (RN) license in the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association.
