RN Manager of Care Management

About the role

AdventHealth Ocala is looking for an RN Manager of Care Management to oversee the daily operations of its Care Management department in inpatient, observation and emergency department settings. The role focuses on discharge planning, care coordination and quality care, working with hospital leadership to improve care flow.

Key responsibilities

• Ensure documentation and compliance for discharge planning, patient choice and other regulatory requirements.
• Ensure staff participate in daily multidisciplinary rounds and take part in interdisciplinary meetings to coordinate care.
• Monitor staff documentation, reports and dashboards to meet company-wide standards and quality care.
• Oversee care managers' full discharge planning from admission to discharge, aiming for timely interventions and avoiding delays and unnecessary readmissions.
• Collaborate with hospital leadership to escalate care delays and ensure daily care progression and throughput.
• Identify process improvement opportunities and manage escalations of delays and complicated cases, working with care providers inside and outside the hospital.
• Educate and reinforce standard work with staff, and handle hiring, onboarding and retention of care management staff.
• Coordinate family and care team meetings and ensure effective transitions of care.

Required profile

• Registered Nurse (RN); Bachelor's of Nursing preferred.
• 3+ years of direct care management experience.
• 2+ years of frontline acute care nursing experience.
• Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred.

The pay range is $78,540.80 to $146,078.40. Positions are subject to Florida Level 2 background screening, and the employer is an equal opportunity employer.
