RN Case Manager – Weekend Days Shift

About the role

Join Cleveland Clinic’s Martin North Hospital in Stuart, Florida, as an RN Case Manager. In this PRN weekend‑day position you will collaborate with inter‑professional teams to coordinate safe transitions of care, advocate for patients and support families in self‑management.

Key responsibilities

• Ensure compliance with accrediting bodies, state codes and CMS Conditions of Participation.
• Promote inter‑professional synchronization throughout the acute episode.
• Complete effective handoffs to the next level of care or community to ensure continuity.
• Proactively assess and mitigate barriers to patients’ transitions of care.
• Develop individualized transitional care plans with patients and care partners.
• Participate and lead in patient rounding activities, huddles and committee meetings.

Required profile

• Graduate from an accredited nursing program (Diploma, ADN or BSN).
• Current RN licensure and Basic Life Support certification.
• At least three years recent RN clinical experience.
• Preferred: BSN, certification (CTM, ACM or CCM) within one year of eligibility.
• Preferred: experience in utilization review, quality or care management, acute care case management, EPIC, and knowledge of CMS regulations.

Required skills
