RN Case Manager – Transitional Care Coordination

About the role

The RN Case Manager will join the Utilization and Quality Assurance team at Florida Weston Hospital, providing patient‑centered transitional care coordination across the continuum of care. This full‑time, day‑shift position supports hospital throughput, complex discharges, and post‑discharge follow‑up.

Key responsibilities

• Conduct holistic patient assessments and develop individualized transitional care plans.
• Coordinate complex discharges, ensure effective handoffs, and mitigate barriers to safe transitions.
• Participate in patient rounding, huddles, and committee meetings to promote interprofessional synchronization.
• Maintain compliance with accrediting bodies, state regulations, and CMS Conditions of Participation.

Required profile

• Graduate from an accredited nursing program with current RN licensure in Florida.
• Minimum three years of recent clinical nursing experience.
• Basic Life Support (BLS) certification.
• Preferred: BSN degree and certification in case management (CTM, ACM, CCM, or AHA) within one year of eligibility.

Required skills

• Proficiency with computer systems and electronic health record documentation.
