RN Case Manager

About the role

The RN Case Manager at Cleveland Clinic Weston Hospital will work with interprofessional teams to coordinate effective transitional care. This full‑time, day‑shift position supports hospital throughput, complex discharges, and post‑discharge follow‑up.

Key responsibilities

• Ensure compliance with accrediting bodies, state regulations, and CMS Conditions of Participation.
• Promote interprofessional synchronization throughout the acute episode.
• Complete effective handoffs to the next level of care or community to ensure continuity.
• Assess, identify, and mitigate barriers to patients’ transitions of care.
• Develop comprehensive, individualized transitional care plans with patients and care partners.
• Participate and lead in patient rounding activities, huddles, and committee meetings.

Required profile

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three years of recent RN clinical experience.
• Preferred: Bachelor’s of Science in Nursing (BSN) and certification (CTM, ACM, CCM or AHA) within one year of eligibility.
• Preferred: Two years of recent experience in utilization review, quality, or care management.

Required skills

• RN licensure.
• BLS certification.
• Knowledge of acute‑care regulatory and accreditation requirements.
