RN Care Manager

About the role

The RN Care Manager will join the multidisciplinary team at Indian River Hospital to provide coordinated, disease‑focused care for high‑risk patients. Working day shifts, the role emphasizes outreach, patient education, and reduction of preventable emergency visits.

Key responsibilities

• Identify patients requiring ongoing care coordination and develop individualized care plans.
• Conduct comprehensive clinical assessments, including medical, behavioral, pharmacy, and social needs.
• Utilize registries and other technological tools to manage high‑risk patient populations.
• Serve as the primary contact for patients, families, physicians, and the care team, facilitating transitions of care and addressing barriers.

Required profile

• Graduate of an accredited nursing program (Diploma, ADN or BSN).
• Current state licensure as a Registered Nurse (RN).
• Basic Cardiac Life Support (BLS) certification.
• Three to five years of nursing experience.

Required skills

• Proficiency with patient assessment and risk‑assessment tools.
• Experience using electronic registries or care‑team coordination platforms.
• Ability to perform comprehensive clinical documentation.
