RN Care Coordinator – Pediatric Cardiology

About the role

The RN Care Coordinator will join the Pediatric Cardiology team at Cleveland Clinic Main Campus, providing longitudinal care coordination for high‑risk pediatric patients. The role focuses on patient outreach, disease management, and reducing unnecessary emergency visits and readmissions.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize patient outcomes.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate patients and families, monitor compliance with care plans, and update plans as needed.
• Serve as liaison and advocate for patients, facilitating transitions of care and connecting to community resources.

Required profile

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BBS) certification through the American Heart Association or Red Cross.
• Three to five years of nursing experience.
• Ability to meet physical requirements, including full range of motion and manual dexterity.
