RN Care Coordinator – Minimally Invasive GYN Surgery

About the role

The RN Care Coordinator will join Cleveland Clinic’s Main Campus Women’s Health team, providing longitudinal care coordination for high‑risk patients undergoing minimally invasive gynecologic surgery. The role focuses on patient outreach, disease management, and reducing preventable emergency visits while enhancing patient satisfaction.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize outcomes for high‑risk patients.
• Identify patients needing ongoing coordination, conduct targeted outreach, and perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate patients and families, monitor compliance with care plans, reassess progress, and update plans of care.
• Serve as liaison and advocate, managing transitions of care across settings and connecting patients to community resources.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• Three to five years of nursing experience, preferably in women’s health or oncology settings.
• Basic Life Support (BLS) certification through the American Heart Association or Red Cross.
• Ability to obtain or maintain a cancer‑specific certification if working in oncology‑related areas.

Required skills
